Where is the input and output on it and negative and positive?

It depends on the voltage regulator. A typical +V IC is 78XX , and a -V is a 79XX where XX represents the voltage wanted.
for those chips the first pin is Vin, the middle pin is GND, and the right most pin is Vout.
