Left it laying on the table, while ON and melting or burning it.
when you are concentrating so hard on getting your solder perfect that the iron melts into the power cord.
I don't solder in shorts any more..... dropped the iron from the table and it stuck to my leg. Not for long as I screamed and ripped it off but left a nasty burn.

things that are meant to melt metal on you body is probably not a good thing
My theory behind this is that alot of student hobbiests spend alot of money on Soldering irons and other tools and we cannot afford to get new tools immediatly if something hhas happened to the old ones.
